S306: TRANSORBITAL APPROACH AS APPLIED TO COMPLEX CRANIAL PATHOLOGY

Transorbital approaches are in their relative infancy. However, they provide an exciting new avenue to expand the realm of keyhole approaches in Neurosurgery. This presentation will explore the authors case series of applying the transorbital approach to complex anterior fossa, middle fossa and intracranial pathology. This series includes resection of 8 en plaque meningiomas, 3 intracranial meningiomas, 2 cavernous epidermoids and clipping of an MCA aneurysm. Philosophical considerations in relation to utilisation of the microscope vs endoscope vs a combination of the two techniques will be explored and series results presented. Surgical videos will be presented.
Conclusions: Transorbital approaches offer an exciting keyhole that can access deep and relatively inaccesible intracranial locations. Acheived resection rates can be excellent and cerebral aneurysm clipping with minimal brain manipulation is possible.
