The surgical procedure presented here involves an orbital exenteration through a transfacial approach, along with endoscopic access to the cavernous sinus and the supraorbital fissure, in a patient with rhinocerebral mucormycosis. The contents of the cavernous sinus, including the internal carotid artery (ICA), were resected, with boundaries extending to the proximal dural ring superiorly and the petrous part of the ICA inferiorly. Necrotic tissue in the middle cranial fossa was identified and removed.
