Chronic allergic fungal rhinosinusitis (AFRS) is typically non-tissue invasive, but in rare occasion, can expand in the paranasal sinuses and cause bony erosion. We present a 34-year-old male with extensive AFRS eroding the anterior skull base and clivus. The video showcases complete bilateral endoscopic sinus surgery, removing all the allergic mucin and diseased mucosa with creation of a neo-sinus cavity in conclusion. The pathology revealed eosinophilic polyps and confirmed allergic type mucin with fungal hyphae.
