This video demonstrates the application of the Contralateral Nasofrontal Trephination (CNT) approach for managing a meningoencephalocele of the lateral recess of the sphenoid sinus. The patient, a 52-year-old female with a history of clear nasal drainage, underwent an endonasal approach assisted by the CNT. The technique provided excellent access to the lesion with minimal morbidity, avoiding the need for a transpterygoid approach. Postoperative outcomes were favorable, with no signs of CSF leak and good cosmesis.
