57 year-old female with history of migraines presented with asymptomatic incidental lesion centered over right ethmoid roof adjacent to olfactory nerve consistent with meningioma. This was found to be growing on surveillance imaging. She underwent endoscopic endonasal transethmoid approach to resection. Customized corridors were utilized to maximize exposure and minimize morbidity by sparing olfaction. The procedure included orbital decompression, drilling of the ethmoid and orbital roofs, tumor resection, and reconstruction.
