V019: EXTREME LATERAL PARTIAL TRANSCONDYLAR APPROACH FOR EXCISION OF A GIANT FORAMEN MAGNUM MENINGIOMA IN A PATIENT WITH DOMINANT OCCIPITAL AND CIRCULAR SINUS

A 62-year-old man presenting with severe progressive headaches was found to have a giant WHO grade 2 meningioma in the foramen magnum (FM) region with severe compression of the brainstem. The patient had a dominant occipital sinus and circular sinus with bilaterally hypoplastic transverse sinus which made tumor resection very challenging through a narrow operative corridor. We demonstrate the operative technique, lessons learned, and strategy to achieve gross total resection.
