A 60-year-old male presented with an MCA pseudoaneurysm after a failed endovascular balloon angioplasty. He also had a cribiform plate arteriovenous fistula with cortical venous reflux. He underwent a superficial temporal artery- double limbed descending lateral circumflex femoral artery interposition graft – temporal/frontal M2 bypass and trapping of the pseudoaneurysm, as well as obliteration of the AV fistula. Postoperative imaging confirmed bypass patency and obliteration of the pseudoaneurysm and fistula.
