V034: MICROSURGICAL CLIPPING OF A LARGE OPHTHALMIC ARTERY ANEURYSM WITH USE OF ENDOVASCULAR BALLOON GUIDE FOR PROXIMAL CONTROL AND ASPIRATION DECOMPRESSION WITH PROPHYLACTIC STA-MCA BYPASS

A 35-year-old female referred for decreased right eye vision was found to have a large 1.1x1.0x1.6 cm right carotid-ophthalmic artery aneurysm. She underwent a angiogram and balloon test occlusion. Given compression of the optic nerve, decision was made to pursue microsurgical clipping with endovascular proximal control and decompression using a balloon guide with aspiration in the right cervical ICA and prophylactic right STA-MCA bypass. Patient tolerated the procedure well and her vision improved.
