This is a surgical video illustrating a ruptured giant thrombosed M2 aneurysm in a young patient. STA-MCA bypass was performed at the acute phase and the aneurysm was excised from the inflow and outflow vessel. Post operative CTA and DSA demonstrated patency of the bypass and no new strokes. Patient was discharged at the rehabilitation centre and at six weeks follow up he had returned to his normal activities.
