This operative video showcases a 5-year-old male with no significant medical history who presented with personality changes and right-side facial palsy. Imaging showed a multi-cystic right temporal mass with the invasion of the mastoid, middle ear, internal auditory canal, temporal and infratemporal fossa. A right temporal craniotomy, trans-zygomatic approach, combined petrosectomy, and facial nerve decompression with closure of the external auditory canal was performed. Post-op his facial nerve palsy improved from III/VI to II/VI.
