Endoscopic endonasal approach for resection of the odontoid process was first described by Kassam et al in 2005. Since then, it has gained popularity in carefully-selected patients over traditional transoral approaches due to its minimally invasive nature and promising clinical outcomes. In this video, we discuss a 30 year-old male with congenital chiari malformation and basilar invagination presenting with 2.5 years of lower extremity weakness undergoing endoscopic transnasal decompression of C1 and C2.
