We describe our experience of an endoscopic endonasal odontoidectomy and pannus removal in a patient with severe CVJ compression in a 73-year-old male presenting with myelopathic symptoms. We discuss various technical nuances of maximizing surgical corridors, predictors of the extent of odontoidectomy, and preventing the need for extending the cervical fusion construct to occiput. Postoperatively, the patient had remarkable improvement in his symptoms. Follow-up MRI at 2 months shows significant ventral reduction of CVJ compression.
