V062: EXTENDED ENDOSCOPIC ENDONASAL APPROACH WITH ODONTOIDECTOMY IN A PATIENT PRESENTING WITH UNSTABLE PATHOLOGICAL TYPE 2 FRACTURE, RETROPHARYNGEAL ABSCESS, AND CERVICAL-MEDULLARY COMPRESSION

We describe a novel extended endoscopic transnasal odontoidectomy in a 56-year-old female chronic smoker presenting with a history of low-grade fever, neck discomfort, and swelling for the past 3 months. On examination, she had mild spasticity in both lower limbs. She underwent odontoidectomy with phlegmon removal and CVJ decompression Postoperatively, there was a remarkable improvement in spasticity. Pathology revealed acute on chronic osteomyelitis. O-C4 fusion was performed post-antibiotics course.
