Dural arteriovenous fistulas (dAVF) are rare pathologies but represent a significant risk to patients from both potential hemorrhage and non-hemorrhagic neurologic deficit. The risk is magnified when cortical venous reflux is present. Complex dAVFs are difficult to treat and require aggressive surgical treatment once endovascular methods fail. We present the technique of dural sinus skeletonization to cure a previously embolized dAVF. Relevant clinical information, preoperative clinical imaging, technical details, and post-operative imaging are reviewed.
