Petrous apex cholesterol granuloma (PACG) is an uncommon, cystic skull base lesion. Although benign, it can expand and cause local mass effect resulting to hearing loss, vertigo, facial palsy and/or diplopia. We present the case of a 23-year-old male with bilateral PACG. The larger left PACG was marsupialized via medial transsphenoidal endoscopic endonasal approach while still clinically silent. A wide drainage to the sphenoid sinus with free mucosal graft was created to prevent recurrence.
