V097: TREATMENT OF A GIANT FUSIFORM VERTEBROBASILAR ANEURYSM IN A HYBRID OR: M2-RAG-P2 BYPASS AND ENDOVASCULAR COILING

Managing vertebrobasilar fusiform aneurysms requires flow reversal and aneurysm thrombosis while preserving critical vasculature. We present a giant fusiform vertebrobasilar aneurysm treated via M2-RAG-P2 bypass and endovascular coiling in a single session. In two-stage approaches, bypass occlusion while awaiting aneurysm takedown is a concern since the bypass competes with the higher-flow basilar while the aneurysm is present. The hybrid OR enabled immediate aneurysm coiling, decreasing the time of competing flow and risk to bypass patency.
