This is a case of a 54 years old male with headaches who was found to have a large left petroclival meningioma mostly locating in the posterior fossa but extending to the Meckel's cave and supratentorial space. A left retrosigmoid transtentorial approach combined with drilling of the suprameatal bone (reverse Kawase) was perfomed. Post-op MRI confirmed gross total resection. Patient was dischargd home without any neurological deficits.
