This is a case of a 19 years old female with a very agressive brain stem cavernoma. She presented in the peripheral hospital with headache, vertigo and photophobia and evidence of bleeding. Within the next two days she had two more episodes of bleeding resulting in her becoming comatose and developing cranial nerve deficits. She underwent emergency right extended retrosigmoid appraoch, splitting of horizontal fissure and resection of the cavernoma through the middle cerebellar peduncle.
