Cavernomas of optic pathway are rare. This is a case of a 18 years old female with visual decline and left homonymous hemianopia. MRI demonstrated a 2cm chiasmatic cavernoma eccentric to the right. A contralateral (left) lateral supra-orbital (LSO) translamina terminalis approach was used to remove the cavernoma. At 4 weeks follow up, patient returned to her normal daily activities. Her visual acuity was almost normal, but the left homonymous hemianopia was persistent.
