Vascular compression of the optic nerve via the internal carotid artery is a rare clinical entity. There are currently no case reports that describe a macrovascular sling transposition technique for this pathology. We present the case of 69 year old woman with severe left vision loss and optic nerve atrophy with an ectatic left ICA. Utilizing a sling transposition technique we were able to effecitvely decompress the optic nerve and prevent further loss of vision.
